MEMENTO.HTN
Memento.HTN is a digital health platform integrated with SMS technology to improve medication adherence for patients with hypertension. By combining accessible technology with practical adherence strategies, Memento.HTN aims to enhance treatment engagement and ultimately improve blood pressure control among at-risk populations.
Product Validation
Initial product dev was completed in 2022, an initial pilot evaluation study showed decreases in systolic blood pressure, enhanced medication adherence and strong user engagement and appeal.
HEART HEALTH
Building on our previous digital health successes, Mobile Heart Care Coordination (MHCC) is a technology-enabled program designed to deliver coordinated post-discharge care for dual eligible older adults with heart failure. This mobile and web-based platform integrates telemonitoring via Bluetooth-enabled devices with mobile phone-based communication to seamlessly connect hospital-based cardiac care teams with community providers. With a focus on real-time vital tracking and educational messaging, MHCC aims to reduce avoidable hospitalizations and emergency room visits for a high-risk population.
Product Validation:
EHG conducted formative evaluations with patients with heart failure and healthcare providers, followed by a pilot clinical trial to assess the feasibility and short-term outcomes of the MHCC intervention. Feedback from these evaluations has been directly integrated to refine telemonitoring functionalities, care coordination workflows, and messaging strategies, setting the stage for a larger, randomized trial.
